Fee-for-Service model with 75/25 split on MSP and uninsured payments; 100% of LFP panel payments and QI project payments paid to physicians; minimum 6 patients/hour guaranteed; flexible payment models offered (FFS, LFP, NTP); Business Cost Premium adds $48/day to billings.

Combination — Longitudinal Family Practice (LFP) payment model. Overhead payable on a monthly equally split basis. Physicians keep 100% of earnings from services performed outside the clinic (e.g., ER, surgical assists, maternity, anesthetics). May be eligible for Rural Subsidiary Agreement (RSA) additional remuneration including a 21.70% retention premium and a $33,629 retention flat fee.

Fee-for-Service; eligible for Rural Subsidy Agreement (RSA) incentives: Recruitment Incentive $10,000 (pro-rated on FTE); Fee Premium 4.62%; Annual Flat Fee $7,159.68; Rural CME starting at $2,200/year (in years 3–4), increasing to $4,400 after 4 years in the community.

Contract. Minimum potential: $332,551 (SHA) + $120,000 (Mosaic) = $452,551. Maximum potential: $385,007 (SHA) + $150,000 (Mosaic) = $535,007. Compensation covers SHA clinical duties (including 187 clinic days, long term care, acute care, home care and an equitable share of after-hours ER coverage).
